This skill set is for people working in various care and support roles in the community services industry who wish to develop skills in supporting people with their self-management of chronic disease.

This skill set has been endorsed by the industry as appropriate for people who hold a qualification at Certificate III level or higher in an area involving the provision of direct client care or support.

Entry Requirements

This skill set is suitable for individuals who hold a qualification at Certificate III level or higher in an area involving provision of direct client care or support.


Program delivery is face to face in a small group training environment.

The unit ‘Support Independence and Well-being’ requires 120 hours of work placement involving direct support work. Additionally, the participant must be able to demonstrate and safely support at least three people to enhance independence and well-being.

Pathways Information

These units provide credit towards a range of qualifications in the CHC Community Services Training Package or HLT Health Training Package at Certificate III or IV level.

The course duration is dependent upon previous skills and experience in the industry area together with applicable credit transfer if this should apply.


Concession fee: $50 (if eligible for funding)

Non-concession fee: $100 (if eligible for funding)

Full fee paying: $597
