Participants will learn how to provide clients with the support needed to plan, implement and sustain lifestyle options that can positively impact their health, while also promoting independence and physical and emotional well being.

Entry Requirements

This skill set is for people working in various care and support roles in the community services industry who wish to develop skills in supporting people with their self-management of chronic disease.

It is suitable for individuals who hold a qualification at Certificate III level or higher in an area involving provision of direct client care or support.


Program delivery is face to face in a small group training environment.

These units provide credit towards a range of qualifications in the CHC Community Services Training Package at Certificate III or IV level.
